DIABETES

The Diabetes Wellness Program provides supervised exercise, education and support throughout your personal journey. All programs are individually designed and supervised by our medical team to ensure integration with your medical professional’s prescribed Care Plan.
 

By integrating exercise into your current care, you can maintain and/or improve your physical well-being, emotional well-being, and overall quality of life throughout your wellness journey.


Our certified health and fitness professionals will coordinate with your healthcare team every step of the way. As changes occur in your health status, we will adapt the program to meet your specific needs.

OUR PROGRAM INCLUDES:

  • A one-on-one consultation with our registered nurse and exercise physiologist

  • A comprehensive health and wellness assessment

  • Individualized, medically-based exercise plans

  • Medically supervised exercise sessions

  • Daily monitoring of progress, blood pressure, heart rate and pulse oximetry

  • Monthly evaluations and summary reports for your referring healthcare provider

  • Unlimited full access to the facility and its amenities

  • Transition to full membership (optional), upon completion of the program
